Earlier this summer, I was delighted to find several helpful video tutorials on YouTube for various cardmaking techniques. This card is called a double slider card and when you pull the ribbon on the top, one panel slides out of the top and another out of the bottom. Here is a picture of it open:
Instead of "reinventing the video tutorial wheel" (RTVTW), I will simply refer you to the one I viewed.I used the Happy Birthday Cuttlebug folder for the embossing on the front of the card and some stamps for the "Happy" and "Birthday" on the top panel.
